We're using OpenNMS at $work (and used it at $work--) for SNMP monitoring and alerting. We haven't set it up for traps yet, that's next year's project, now we're just thresholding on monitored values (disk space, cpu, etc). Takes a bit of effort to get it setup, but the online documentation is improving, and it's quite solid once it's running.
